Delivered Architecture & Deliverables
Technologies & Infrastructure Stack
Home MD Health is a US-based healthcare provider offering in-home physician services, clinical care and chronic-care management. The platform was developed to create a clearer digital pathway for patients, families and partner care facilities seeking healthcare services.
My contribution focused on full-stack architecture, healthcare workflows, API integrations, responsive interface development and scalable cloud infrastructure.
The Challenge
In-home healthcare involves several participants, including patients, family members, physicians, care facilities and external service providers.
The platform needed to organize these interactions into one accessible digital experience while supporting:
Patient onboarding
Healthcare-intake workflows
Service exploration
Appointment and care requests
Scheduling coordination
Patient-information exchange
Third-party healthcare integrations
Desktop, tablet and mobile access
The primary challenge was maintaining a simple patient experience while building a scalable technical foundation for healthcare operations.
Project Goals
The platform was designed to:
Simplify access to in-home healthcare services.
Create structured patient-onboarding workflows.
Improve the medical-intake experience.
Support appointment and care-request coordination.
Enable integration with external healthcare services.
Provide a responsive experience across devices.
Establish a modular foundation for future development.
Strategy and User Journey
The platform was structured around the complete healthcare-access journey:
Discover → Understand → Onboard → Submit Information → Request Care → Coordinate
This approach reduced unnecessary complexity and helped users move naturally from learning about available services to beginning their care journey.
Patient Onboarding
A structured onboarding flow was designed to collect the information required to begin the patient journey without overwhelming the user.
Healthcare Intake
Digital intake workflows organize patient and service information before care coordination begins.
Appointment Coordination
The platform supports workflows for requesting healthcare services, scheduling appointments and coordinating the next steps in the care process.
API and Integration Layer
RESTful APIs connect the patient-facing platform with scheduling workflows and relevant third-party healthcare services. The integration layer was structured to support maintainability and future expansion.
Responsive Experience
The interface was optimized for patients, families and partner facilities accessing the platform through desktop, tablet and mobile devices.
Cloud-Ready Architecture
The platform was designed with modular services and scalable infrastructure, providing a foundation for future healthcare services, integrations and operational workflows.
My Contribution
My responsibilities included:
Contributing to the modular full-stack architecture.
Structuring patient onboarding and intake workflows.
Developing RESTful API integrations.
Supporting appointment and scheduling functionality.
Building responsive patient-facing interfaces.
Integrating relevant third-party services.
Creating reusable application components.
Supporting scalable cloud infrastructure.
Improving performance, reliability and maintainability.
Results and Product Impact
The delivered platform created a connected digital foundation for Home MD Health’s patient-facing services.
It supports:
Clearer access to healthcare information
Structured patient onboarding
Organized medical-intake workflows
Appointment and service coordination
Third-party healthcare integrations
Responsive access across devices
Future platform growth
Quantitative performance figures should be added only when verified analytics or client-approved project records become available.
